Program Description:

Referrals are made by rhe nursing home staff or requested by PCP or families to a Nursing Home Program nurse; triage is done. Physicians are at the nursing homes one half-day each week to evaluate new patients and follow-ups. PCP, families and nursing home staff are included in the plan around treatment. The physicians and a nurse are available for emergencies. In-services are scheduled on a regular basis. The staff collects data and researches the outcome of interventions. Clinical drug trials for memory, depression and behavior disturbance are conducted at the nursing homes.
