Program Description
The Geriatrics Assessment Center provides comprehensive outpatient and in-hospital interdisciplinary consultation for complex geriatric problems. The emphasis is on the prevention and early detection of chronic disease, health management strategies for chronically ill patients and families, and the care of frail, dependent, elderly patients.
The Center provides consultation on the care of the non-institutionalized geriatric patient and the preservation of patient independence. Risk identification and interventions to avoid or postpone crises that precipitate hospitalization and institutionalization are stressed. Hypertension, osteoporosis, depression and substance abuse receive special emphasis. The team consists of a geriatric medical specialist, nurse practitioner, social worker and pharmacy consultant. When appropriate, coordination is provided with neurology and/or geropsychiatry.
Geriatric-specific methods of diagnosis and treatment are designed to be operationalized 6y the referring physician in order to optimize the utilization of community-based health care resources.
